(1) How many service providers are currently contracted to provide services in the lower South West under the National Disability Insurance Scheme (NDIS) My Way Trial? (2) In relation to each of the service providers identified in (1): (a) what services are offered by the provider (specify); (b) does the service provider have an office in the lower South West and, if yes, state the location of the office; (c) what specialist mental health or psychosocial services does the provider offer (specify); (d) how many lower South West NDIS My Way Trial participants does the provider currently have listed as clients; and (e) which service providers currently have no lower South West NDIS My Way Trial participants listed as clients?

1) Fifty nine (59) service providers on Disability Services Commission panel contracts  identify as being available to provide services in the Lower South West WA NDIS My  Way trial area. 2) a) Service providers are approved onto Panel Contracts for Individualised Funding and/or Disability Professional Services.
Individualised Funding can include supported community living, personal care and support, community access, recreation, support into employment and social inclusion. Disability Professional Services include physiotherapy,   occupational therapy, psychology, speech pathology and a range of specialist disability services. A list is tabled which identifies the type of service provided by each service provider.  [Please see tabled papers no…]
