The Housing Options Assessment Form is not an application for public housing assistance. It is used to triage clients seeking housing or other assistance and advise them of assistance options they may be eligible for based on their current circumstances. This data is not captured by the Department of Communities’ internal reporting systems.
If assessed as being eligible for public housing assistance, an applicant will be encouraged to complete a public housing application.
