I refer to recent booster vaccinations for hepatitis B and tetanus, undertaken at the Kalbarri District High School, for year seven students on Thursday 9 September, and I ask -
(1) Were parents of all students in year seven informed by correspondence of the vaccinations scheduled for 9 September?
(2) If no to (1), why not?
(3) If yes to (1), were all parents required to return signed permission slips to the school?
(4) If no to (3), why not?
(5) Can the Minister confirm that vaccinations for year seven students were carried out at Kalbarri District High School earlier in the year, and that signed permission slips were required to be signed by parents on that occasion?
(6) Can the Minister verify that a year seven female student, not enrolled earlier in the year, was given booster shots on 9 September without parental permission?

(1) The school sent the consent form to parents at the beginning of the school year. The pre-vaccination form included in the vaccination consent form, informs parents who give their consent that it may take a number of weeks for the vaccines to be delivered to the school and administered by the community health nurse.
The school newsletter advised parents that the immunisations would take place on Tuesday, 7 September 2010. However, due to unforeseen circumstances, the vaccinations were postponed to 9 September 2010.

(5)  Yes.
(6)  No. A thorough check of school and health records has been made and there is no evidence that any student was immunised without a signed consent form.
