(1) No formal complaint is recorded as having been received by the then Builders Registration Board in relation to Len Fode Roofing.  However, information was received on 15 March 2011 that indicated that a possible breach of the
Builders' Registration Act 1939
had been committed.
(2) The information received indicated that Mr Len Fode, trading as Len Fode Roofing, may have contracted for and performed building work consisting of the reroofing of a commercial premises with a value in excess of the prescribed amount of $20,000 whilst not being a registered builder.
(3) An investigation was undertaken in relation to the matter.  As a result of the investigation a formal warning was issued to Mr Fode, for performing building works whilst not registered as a builder, contrary to section 4(1)(A)(b) of the
Builders' Registration R Act 1939
.
(4) Not applicable, see response to (3).
