(1) For each year of 2010, 2011 and 2012, how many children and adolescents were referred to SW CAMHS for mental health assessments?
(2) For each year of 2010, 2011 and 2012, how many assessments were undertaken?
(3) For each year of 2010, 2011 and 2012, how many of the children and adolescents assessed were —
(a) under the age of 12 years;
(b) 12 years up to 16 years;
(c) 16 years up to 18 years; and
(d) 18 years up to 24 years?

The Psychiatric Services Online Information System (PSOLIS) which is a high-secure electronic medical record, used in public mental health clinical settings defines any activity related to processing a referral and assessing the patient is classified as "Assessment". As such, some of the children and adolescents referred may have more than one assessment depending on presentation and clinical need.)
