south west region model of palliative care in the home deliver the same
services as the Perth metropolitan area model of palliative care?
(2) If no to (1), what are the
differences in the two services?
(3) What changes,
if any, have recently been made or have recently been notified to be made to
the delivery of the community palliative care nurse service in the south west
region?
(4) What are the reasons for these
changes?

(1) The model of
palliative care in the south west delivers the same types of services as in
metropolitan Perth. Patients in both settings have access to inpatient
specialist palliative care, provided by St John of God Bunbury Hospital, and
community palliative care nursing services. The disciplines employed in the
Bunbury community palliative care team are equivalent to the disciplines
employed in metropolitan Perth by Silver Chain—that is, specialist
palliative care nursing, social work and access to advice from a palliative
care physician. In addition to the inpatient service at St John of God Bunbury
Hospital and the Bunbury community palliative care team, the south west
regional palliative care team provides specialist palliative care advice and
support—nursing and social work, with palliative care physician input—to
other hospitals in the region to enable them to meet the needs of palliative
care patients. This is the same model that is employed in all rural regions of
WA.
(2) Not applicable.

change has been to Bunbury community palliative care out-of-hours telephone
advice, which is now provided through Bunbury Hospital, and this is now in line
with the model across the south west.
