Under a national agreement, the Western Australian public health system provides de-identified information on medical negligence claims to the Australian Institute of Health and Welfare. That organisation publishes regular reports on its website. Data on all potential claims and actual claims are reviewed and analysed. If the potential or actual claims arose from a preventable act of negligence or omission then remedial action is initiated as quickly as possible. Only two percent of adverse clinical incidents give rise to a claim of medical negligence.
