The Neurosciences and the Senses Network has developed the Model of Stroke Care for WA guided by the Clinical Services Framework and based on best practice evidence. The Model of Stroke Care for WA focuses on the provision of optimal acute and rehabilitation management of stroke patients. This includes the transition to a return to home with appropriate community based care. The State Health Executive Forum (SHEF) endorsed the model in February 2006. A broad stakeholder group, consisting of area health service representatives, allied health, carers and consumers have been brought together and this group is currently developing an implementation plan for the model. The prevention of Stroke is part of a broader health promotion strategy to reduce the incidence of Chronic Disease both in Western Australia and nationally.
