(a) how many applications were submitted by the Member for Vasse for the period between 21 June 2012 and 11 September 2012;
(b) how many applications were approved for payments to the Member for Vasse for the period between 21 June 2012 and 11 September 2012;
(c) what was the amount paid for each individual application by the Member for Vasse;
(d) what dates were covered for each individual application by the Member for Vasse; and
(e) since 1 January 2012, has the Member for Vasse refunded any payments other than the refunded payment of 21 June 2012, and if so:
(i) how much;
(ii) for which date or dates was the payment refunded; and
(iii) on what date or dates was the repayment made?
