(1) Why wasn't the significant cost of sorting, reformatting, re-covering records, their amalgamation with Fremantle Hospital records, their storage and the accessing of records, included in the brief to the consultants for the feasibility study of the move to Kaleeya Hospital by SMAHS CEO Linda Smith?
(2) Was SMAHS CEO Linda Smith aware there was never sufficient storage space at Kaleeya Hospital to store the medical records from Woodside Hospital?

2. Kaleeya is part of the Fremantle Hospital and Health Service. All patient records are stored centrally.
