I refer to the Department of Regional Development and Lands contract, reference number A1775943, for a survey of country age pension fuel card holders within Gingin and Mandurah, and I ask:
(a) what is the total cost of the contract;
(b) what is the name of the company awarded the contract;
(c) on what date was the survey first conducted;
(d) when will the survey be completed;
(e) when will the results of the survey be collated;
(f) what was the nature or purpose of the survey;
(g) what were the postcodes of the card holders surveyed;
(h) will the Minister table a copy of the survey or the survey questions and, if not, why not;
(i) has the Department previously conducted similar country age pension fuel card holder surveys; and
(i) if yes, how many similar surveys have been conducted;
(ii) if yes, on what date(s) were the surveys conducted;
(iii) if yes, what has been the cost of each individual survey; and
(iv) if yes, what were the postcodes of the card holders surveyed for each individual survey?

(b) Coakes Consulting.
(c) 5 April 2012, as advised by the contractor.

(e) The survey results are currently being collated.
(f) To establish the mode, method and purpose of travel of the survey sample group.
(g) 6211, 6503, and 6566, Postcode 6566 (Toodyay) was selected during the course of the survey to provide statistical rigor to sample sizes.
(h) No, the survey process is incomplete.
(i) Yes;
(i)  One;
(ii)  March 2010 to September 2010;
(iii)  The survey was conducted as a component of an Evaluation of the Country Age Pension Fuel Card. The survey component of the Evaluation was not costed separately. The total cost of the evaluation was $89 778.68 excluding GST.
(iv)  Specific cardholder postcodes were not collected as part of the survey. The 2010 evaluation was conducted in the regional centres of Northam, Broome, Albany and Geraldton.
