(1) Can the Minister advise when the presence of the 'dead-leg', which formed part of the water supply ring main for Queen Elizabeth II Hospital, was first brought to the attention of the senior management team of the North Metropolitan Health Services (NMHS)? (2) Given that the 'dead-leg' section of the water supply ring main for the hospital had been in-active for some decades, had the North Metropolitan Health Service ever recommended to have it removed or shut off through its own internal maintenance and operational procedures, and if so, when was this advice last provided to senior management and why was it rejected? (3) Was Strategic Projects, who were managing the construction of the Perth Children's Hospital, notified by NMHS about the presence of the 'dead-leg' prior to the contractor connecting the new hospital water system into the QEII ring main?
