(1) Two. (2) The first occurred on 6 January 2009 at the KCGM Fimiston pit in Kalgoorlie, and the victim was employed by Kalgoorlie Consolidated Gold Mines Pty Ltd. The second occurred on 24 February 2009 on the BHP Billiton iron ore railway, and the victim was employed by BHP Billiton Iron Ore.
