(a) Amendment No. 62 to the Town of Port Hedland Town Planning Scheme No. 5 sought to rezone the former Port Hedland Hospital site from 'Community-Health' to 'West End Residential' with a density code of R160. It was approved on 19 December 2013.
(b - c) I declared a potential conflict of interest due to BHP Billiton Ltd shares in which I have an interest through my late mother's estate. As BHP Billiton Ltd was a third party to the Amendment, Minister Hames agreed to act on my behalf in the decision making and approvals process to avoid any perceived or potential conflict of interest.
