RUBY NICHOLLS-DIVER — SUICIDE
I refer to the suicide of Ruby Nicholls-Diver after being discharged from Fremantle Hospital’s secure psychiatric unit. (1) When was the hospital first notified that Ruby Nicholls-Diver tried to kill herself? (2) Why was Ruby Nicholls-Diver discharged from the Alma Street Centre, given that she told staff that she felt unsafe and that she was in a suicidal state? (3) Who authorised the discharge of Ruby Nicholls-Diver? (4) How many vacancies were there at the Alma Street Centre on the day that Ruby Nicholls-Diver was discharged? (5) Can the minister provide a schedule of the vacancies at the Alma Street Centre for the month of March 2011; and, if not, why not? Hon HELEN MORTON
