I refer to an incident involving a young man and a forklift at a work site in Welshpool on 11 September 2009. (1) When did WorkSafe visit the site following the incident? (2) Is WorkSafe producing a report into this incident? (3) If yes to (2) — (a) when was this report commenced; (b) if it has been completed, when was it completed; (c) if it has not been completed, when will it be completed; and (d) when will this report be made public?
