(a) how many applications were submitted by the Member for Vasse for the period between 11 September 2012 and 6 November 2012;
(b) how many applications were approved for payments to the Member for Vasse for the period between 11 September 2012 and 6 November 2012;
(c) what was the amount paid for each individual application by the Member for Vasse;
(d) what dates were covered for each individual application by the Member for Vasse; and
(e) since 14 September 2102, has the Member for Vasse refunded any payments and, if so, for what amount and for what date(s)?
