I refer to the Bellaisle mine at St Ives, owned by Goldfields, and operated by the contract company GBF, and ask -
(1) Has it been reported to the Department of Mines and Petroleum (DMP), that within the last eight weeks a shot was fired in the underground mine at Bellaisle with the effect of causing uncured pastefill to run out of the stope and into the decline to the point whereas a concrete fill to the approximate cost of $35 000 was necessary to stop the pastefill from continuing to run into the decline?
(2) If no to (1), will the Minister direct the DMP to carry out an urgent investigation into this matter?
(3) If no to (2), why not?
(4) Does the Minister and DMP inspectorate regard the incident referred in (1), above as extremely serious?
(5) If no to (4), why not?
(6) If yes to (4), why?
(7) Have any breaches or non compliance issues of the
Mines Safety and Inspection Act 1994 and Regulations 95
been breached in relation to the incident referred to in (1), above?
(8) Will the Minister state, what is the maximum penalty applicable for such breaches?
(9) Will the registered manager, or any other person at GBF or Goldfields mine, be prosecuted for the incident referred to in (1), above?
(10) If no to (9), why not?

(2) The mine has been contacted and the Underground Manager has confirmed that there was no incident such as that described in your question at the Bellaisle Mine within the last eight weeks.

(4) I am informed that there was no such incident, but an incident involving unplanned and uncontrolled movement of large quantity of fluid/fill is considered to be a potential hazard.
(5) Not applicable
(6) Any uncontrolled movement of large quantity of fluid/fill has the potential to cause harm.
