(1) The health service agreement between the Health Department of Western Australia and health services includes the purchasing and funding of community health services.  Community health services are provided from various settings and facilities throughout the State. (2) The Fremantle Hospital and health service and the Rockingham-Kwinana health service have existing service agreements with the Health Department to provide community health services.  The services are delivered from numerous facilities, including facilities owned by the Health Department, local government and other community organisations. (3) The annual service agreements between the Health Department and the health services expire on 30 June 2001. (4) Yes, service agreements are renewed commencing on 1 July 2001.
