(1) How many meetings were held and how many phone calls were
made to discuss these concerns?
(2) Did DAFWA
make any file notes or other written records of the meetings or telephone calls; and, if yes, how many
written records did it make?
(3) How many letters and how many emails have been sent and
on what dates were they sent?
(4) On what date was the minister first made aware of the
concerns?

I thank the member for some notice of this question.
(1) A number of phone calls and meetings occurred from early
2014 and are still ongoing.
(2) Yes, but given the time available we have not been able
to access them.
(3) Approximately 17 documents and
160 emails were sent over an extended period.
(4) Early 2014.
