For each of the following Department of Housing Homeswest maintenance job orders JO 4631496; JO 4622621; JO 4636274; JO4696364; and JO 4624199, I ask -
(a) on what date was the job order submitted to the Department of Housing for payment and on what date was the job order paid by the department;
(b) who informed the department that the job order was fraudulent or non-compliant, and when;
(c) was the job order payment refunded to the Department of Housing, and if yes on what date;
(d) was any contractor or subcontractor associated with the job orders above, or any of the other eight job orders identified by the same audit, terminated by the head contractor; and
(e) if yes to (d), which contractor or subcontractor was terminated, at on what date?

(a) JO 4631496 was submitted to the Department on 13 August 2010 and paid on
18 August 2010;
JO 4622621 was submitted to the Department on 21 September 2010 and paid on 22 September 2010;
JO 4636274 was submitted to the Department on 3 September 2010 and paid on 8 September 2010;
JO4696364 was submitted to the Department on 15 September 2010 and paid on 13 October 2010; and
JO 4624199 was submitted to the Department on 5 October 2010 and paid on 13 October 2010.
(b)The jobs were identified as being non-compliant by the Department in its standard desktop auditing program and confirmed by a physical inspection by a Department Quality Assurance Auditor in March 2011.
(c) No payment has been received for these job orders to date.
(d) No, a sub-contractor was not re-engaged.
