I refer to
the Corruption and Crime Commission's Report into
the Misuse of a WA Police Information System , in which the CCC formed an
opinion of serious misconduct in relation to Officer A's access of the WA police
incident management system on dozens of occasions.
(1) What
disciplinary action, if any, has been taken against Officer A by WA police?
(2) Has Officer A been charged in relation to this
matter?
(3) If no to (2), what factors contributed to the
decision not to issue a criminal charge?
(4) Have there been other incidents of
unauthorised access that have been investigated by the Internal Affairs Unit?
(5) If yes to (4), when was the most recent
investigation?

confidence disciplinary process.
