I refer motor vehicle accidents at several intersections in Baldivis, and ask: (a) what was the total number of accidents, including a breakdown of how many (if any) fatalities, at the intersection of Mandurah Road and Fifty Road in the following years: (i) 2014; (ii) 2015; and (iii) 2016 to date; (b) what was the total number of accidents, including a breakdown of how many (if any) fatalities, at the intersection of Kulija and Millar Road in the following years: (i) 2014; (ii) 2015; and (iii) 2016 to date; and (c) are any of the above intersections formally recognised by Main Roads as black spots, if so, which intersections?

(ii)        no reported crashes
(iii)       no reported crashes to date
(b)        (i)         no reported crashes
(ii)        1 crash with no fatalities
