I refer to the Tactical Response Group (TRG) bomb technician injured in a detonator explosion earlier this year and ask:
(a) what investigations are currently being conducted; and
(i) if so, what is the nature of the investigation/s;
(b) are the Western Australia Police co-operating with Worksafe and are those investigations operating in parallel;
(c) are Police internal affairs involved in the investigations;
(d) has the incident been referred to any other agency;
(e) how long have TRG and bomb squad officers been issued with polyester garments instead of cotton, when polyester is flammable; and
(f) what directives have been issued to officers about wearing polyester clothing since the incident?

(a) There is a current investigation into the incident by a senior officer of the Western Australia Police.
(i) The investigation is examining the circumstances and cause of the injuries to the officer.
(b) Worksafe was advised of the incident. WA Police are not able to comment on behalf of Worksafe.
(c) No.
(d) Yes. The matter was reported to Worksafe on 4 August 2011.
(e) Polyester T shirts have been issued to TRG officers since September 2006.
(f) On 15 August 2011 TRG officers were issued the following instructions concerning the polyester T shirts:
· Polyester T Shirts are not to be worn underneath the flash proof overalls when attending operational incidents, or at training / other activities at which they may be exposed to flames.
· Polyester T shirts may be worn for physical training or general fatigue duties.
· In accordance with Standard Operating Procedures, the flash proof overalls on issue are to be fastened to the top of the neck with no parts of any undergarments exposed, and sleeves are to be worn to the wrist and not rolled back so as to expose any part of the forearm when attending operational incidents, or at training / other activities at which the officers may be exposed to flames
