(1) By surgery type, as at 1 October 2012, how many people were on the elective surgery waiting list?
(2) In relation to (1), how many people on the elective surgery waiting list have been waiting —
(a) less than 12 months;
(b) between 12 months and 18 months;
(c) between 18 months and 24 months; and
(d) more than 24 months?
