With regard to question on notice No. 1975 of May 4 2004, I ask:
(1) Did the Forest Products Commission or Hansol Australia Pty Ltd instruct Bailey Fertiliser Company to have any hazard warnings issued with the supply and use of the special order of fertiliser pills (box code number 242502) used by the contractor at Dunnet Share Farming in mid 2002?
(2) If not, why not?
(3) Which of the following authorities -
(a) the Forest Products Commission;
(b) Hansol Australia Pty Ltd; or
(c) the Collie office of the Conservation Land Management,
authorised the Bailey Fertiliser Company to add copper sulphate, organochlorine and organophosphate to the fertiliser pills described in question (1)?
(4) Will WorkSafe further investigate this matter?

The FPC supplied this information to WorkSafe during May 2004. The FPC is not aware whether the WorkSafe enquiry is continuing and details should be sought from the Minister for Consumer and Employment Protection.
