1. The waiting period required is variable dependent upon several factors, including type of offence that leads to the disqualification/suspension of the driver's licence, as well as any previous convictions for alcohol related driving offences.
2. Section 76(5)(a)(i)&(ii) of the
Road Traffic Act 1974
, provides the conditions that can be imposed.
Standard conditions generally imposed by Magistrates, are set out on the back of the court order form.

4. Yes. There is nothing in Section 76 of the
Road Traffic Act 1974
that prohibits a person applying for an extraordinary licence, should they have been issued with one previously.
5. No additional conditions are applicable for a second or subsequent application for grant of an extraordinary licence.
