(1) The Mental Health Commission will provide funding of $2,917,457 in 2012/13 to six non-government agencies for community support and accommodation services in the South West. These services, along with community based clinical services and local services provided by general practitioners can be accessed by people being discharged from BunburyRegionalHospital.
The following support services are provided to patients on release from the unit:
If the Consultant Psychiatrist and the multidisciplinary team of the unit feel that post-discharge follow up with the Community Mental Health Team is required, then a referral is made by the inpatient unit to the intake triage team of the Community Mental Health Clinic.
If the patient does not require a referral to the Community Mental Health Clinic and are discharged to the care of their general practitioner following their inpatient admission, then the patient receives a contact from the discharge liaison nurse of the inpatient unit within 7 days of their discharge from the inpatient unit.
Some patients, depending on their clinical needs, are also referred to non?government organisations (NGOs) for example, South West Drug and Alcohol team, Pathways NGO, Lamp Inc NGO etc.
All patients also have access to Rural Link which provides after hours support.
(2) Patients are provided with information packs which contain information in regard to local mental health services, Rural Link, and any other mental health resources appropriate for their clinical needs. If they are referred to an NGO, information in the form of brochure is provided to the patient and often the referred NGO will provide inreach to the unit prior to the patient being discharged from the unit.
(3) Where possible, the inpatient staff member makes these appointments.
(4) Non-government organisations which provide community support services do not typical operate a waiting list process and are not required to provide the Mental Health Commission with this information. Referrals are prioritised based on immediate needs and available resources.
Wait times for South West Community Mental Health Service: generally within 7 days of discharge from the unit.
(5) If the patient does not require a referral to the community mental health clinic and is discharged to the care of their general practitioner following their inpatient admission, then the patient receives a contact from the discharge liaison nurse of the inpatient unit within 7 days of their discharge from the inpatient unit.
