(b) On the 23 December 2008, the Commonwealth and WA Governments announced that a jointly conducted inquiry would be undertaken into the effectiveness of safety regulation for upstream petroleum operations with a focus on the incident at Apache Energy Ltd's facilities on Varanus Island.
(c) Industry Working Group on Approvals
(d) Nil
(2) (a) Not applicable
(b) The inquiry will commence late January 2009 and a report is expected within 3 months of the commencement of the inquiry.
(c) The Industry Working Group on Approvals was formed via ministerial announcement on 17 November 2008 and completion is due end April 2009.

(b) The inquiry will be conducted by a two-member expert panel i.e. David Agostini (State appointed) and Kym Bills (Commonwealth appointed)
(c) Chairman - Hon Peter Jones AM, former Resources Minister in the Sir Charles Court Government (retired).

(b) Funding for the inquiry will be met by the Commonwealth Government and capped at $1 million.
