the name and/or number of the Western Australian legislative regulations form

Workers' Compensation and Injury Management Act 1981, they are required
to issue a form 3B—insurer's notice that liability is disputed.
(2) No. I am
advised that Mr Barkla's insurer was not required to issue a form 3B in
relation to his claim.
