(1)-(3) During 2003/04 an online incident notification system (commonly referred to in schools as the critical incident database) was implemented. This system builds on the earlier fax system of reporting. a) All schools are expected to submit notification of any event or issue of a critical or highly contentious nature. When the principal enters the incident into the online database, a description is given which may or may not include the term 'bullying' and may or may not include details relating to medical attention sought or received. b) Schools' reports are automatically submitted to central office and to the relevant district office for appropriate support and/or advice. Schools are required to provide information to the district director about students who have been suspended and/or excluded which may or may not be a result of an incident. During Term 2, 2006 the new version of the online incident notification system will be implemented. This system provides for greater consistency in relation to data entry. The following tables provide an overview of the incidents submitted online that reflect the terminology used in parts (1) - (3). It should be noted that incidents submitted may not always use the specific terminology used in the Parliamentary Question.
