I refer to the tragic loss of three lives on 27 September 2023 arising from a motor vehicle accident on Sixty Eight Road in Baldivis, and I ask: (a) at what time on 27 September 2023 was assistance first requested from the Department of Fire and Emergency Services (DFES) in relation to this incident; (b) by what means was assistance requested and by whom; (c) at what time on 27 September 2023 were each of the following Brigade, Group or Unit (BGUs) requested: (i) Rockingham 1st; (ii) Rockingham 2nd; (iii) Mandurah First; (iv) Hope Valley; (v) Secret Harbour; (vi) Baldivis VFES; and (vii) Any other BGU; (d) at what time on 27 September 2023 did each BGU respond; (e) at what time on 27 September 2023 did each BGU arrive at scene; (f) for each BGU identified in (c), what is the distance by road from the BGU location to the incident location; (g) has any formal or informal request been made to add a road crash rescue capability or profile to the Baldivis Volunteer Fire and Emergency Serives (VFES); (h) on what dates was each request made; and (i) for each instance identified in (h) who made the request and what was the outcome of each request?
