I refer to the Ombudsman’s report A Review of Family and Domestic Violence in Western Australia, which identified that a significant proportion of cases, including some assessed as high risk, resulted in no further action, and I ask: (a) please table the amount of family and domestic violence cases resulted in no further action in each of the past three years; (b) of those cases in (a) how many were assessed as high risk of serious harm; (c) what criteria or decision-making framework is applied when determining that no further action is to be taken; and (d) what oversight, review or escalation mechanisms exist in relation to such decisions?

(a - b) The information sought is contained in individual incident reports and is not available in an easily reportable format.
(c - d) The Family and Domestic Violence Response Team (FDVRT) Operating Procedures guide responses to reports of family and domestic violence. Existing governance mechanisms providing oversight of the FDVRT model include an Executive Steering Committee, which is supported by an Operational Working Group. Both groups comprise representatives from partner agencies.
