I refer to the 32 investigable child deaths currently (as advised to me by the Ombudsman in correspondence of 19 April 2011) being reviewed by the Ombudsman’s office, and I ask -
(1) What was the date of the death for each of those children?
(2) How many of these child deaths were referred under each of the following criteria -
(a) in the two years before the date of the child’s death, the Chief Executive Officer (CEO) [of the Department] had received information that raised concerns about the wellbeing of the child or a child relative of the child;
(b) in the two years before the date of the child’s death, the CEO, under section 32(1) of the
Children and Community Services Act 2004
, had determined that action should be taken to safeguard or promote the wellbeing of the child or a child relative of the child;
(c) in the two years before the date of the child’s death, any of the actions listed in section 32(1) of the
Children and Community Services Act 2004
was done in respect of the child or a child relative of the child;
(d) protection proceedings are pending in respect of the child or a child relative of the child; or
(e) the child or a child relative of the child is in the CEO’s care?
