(1) For the 2023/24 financial year have any “Request to Attend Event” forms been submitted by Ministerial staff (at or above PSGA Level 5 or equivalent), and if so: (a) Have these forms been submitted to the DPC Integrity Unit: (i) If no to (1(a)) why not; (b) If yes to (1(a)) please detail each event attended by staff; (c) For each event named in (1(b)) what date was each event held; and (d) For each event named in (1(b)) and dated in (1(c)) please detail when the "Request to Attend Event" form was submitted to the DPC Integrity Unit? (2) For the financial year 2024/25 to date have any “Request to Attend Event” forms been submitted by Ministerial staff (at or above PSGA Level 5 or equivalent) and if so: (a) Have these forms been submitted to the DPC Integrity Unit: (i) If no to (2(a)) why not; (b) If yes to (2(a)) please detail each event attended by staff; (c) For each event named in (2(b)) what date was each event held; and (d) For each event named in (2(b)) and dated in (2(c)) please detail when the "Request to Attend Event" form was submitted to the DPC Integrity Unit?
