They are being supported by the clinical key workers in the community. The key workers are clinicians and comprise clinical nurse specialists, community mental health nurses, social workers, clinical psychologists or occupational therapists. (2) Ongoing clinical risk reviews are conducted for each patient. Each patient is regularly monitored and supported by the clinical key worker.
