(7) There is no system that specifically tracks community deaths following discharge from the Emergency Department. Bunbury Hospital will only become aware of a death if the death is reported back to the hospital by a third party. Bunbury Hospital is aware of the following deaths occurring within 24 hours of the deceased person being released from the Emergency Department at the South West Health Campus:
(a -d) Not available.

(8) 2005 - Investigation status of the one death known to Bunbury Hospital is not known.
2006 - No deaths known to Bunbury Hospital.
2007 - Two of the deaths known to Bunbury Hospital were investigated (one by the hospital, one by the State Coroner). The third death was expected, and therefore was not investigated.
(9) Advice as to the outcome of an investigation into a particular case would constitute a breach of confidentiality.
