With regards to A1 Minerals operations, at the Brightstar mines of Beta and Alpha near Laverton, I ask -
(1) Did the Mines Department visit these mines in November or late 2009?
(2) Were issues of bullying, harassment and safety issues raised with the Departmental officer during that visit?
(3) If yes to (2), -
(a) what were the issues raised; and
(b) what action or investigation has been undertaken by the Department, as a result of these issues being raised?

(b)        Not applicable.  However, based on the inspection, two Improvement Notices were issued dealing with:
1.    dust associated with drilling, and
2.    the bund around an open pit.
