I refer to the Department of Mines and Petroleum Resources Safety Division, and I ask in relation to mining, safety and health for the period 2012-13, and 2013-14 to date: (a) how many complaints about workplace health and safety were received; (b) how many notifiable incident reports were received; (c) how many investigations were undertaken; (d) how many inspections and audits were carried out; and (e) in relation to (c), what were the outcomes of the investigations?

The Department of Mines and Petroleum advises:
(a)
There were 1 507 initial lost time and restricted work injuries reported for 2012, and 1 409 for 2013 in mining and exploration operations.
The table provides the nature and number of injuries [See Tabled Paper].

All the injuries were either lost time or restricted work injuries where the injured persons were disabled by the accident from performing his or her duties of employment
as they were being performed at the time the accident occurred.

13 fatal and serious investigations were undertaken
2013-14:
23 fatal and serious investigations were undertaken

1 827 inspections and 179 audits were undertaken
2013-14:
1 463 inspections and 146 audits were undertaken
(e)
Outcomes of investigations meet the Department's prosecution and enforcement policy. During the investigation process prohibition and improvement notices are issued by inspectors as required and learnings are shared with the mining industry in the form of safety bulletins and significant incident reports when facts are established. In 2012-13 nine investigations were referred to the State Solicitor's Office (SSO) and in 2013-14 one has been referred to the SSO.
