I refer to the Norseman Gold Operations, the OK Mine, Harlequin Mine, and Bullen Mine, and ask -
(1) Has the Department of Mines and Petroleum (DMP) received an incident report in relation to a collision between an underground loader and twin boom jumbo drill at the Harlequin Mine in the last 30 days?
(2) If yes to (1), -
(a) has the DMP carried out a full investigation into this matter; and
(b) what was the factual circumstances which led to this collision occurring?
(3) If no to (1), will the Minister request that an investigation take place concerning this matter?
(4) In relation to the collision of an underground loader and twin boom jumbo within the last 30 days, did management of the mine report to the DMP that the bogger was tramming down forwards when the collision happened?
(5) Has the DMP received an incident report from the Harlequin mine concerning a light vehicle whose steering box was completely rusted off the chassis which caused the vehicle to hit the wall and the ute body to break in half from excessive structural rust?
(6) If yes to (5), -
(a) has the DMP carried out a full investigation into this matter; and
(b) what were the factual circumstances which contributed to this incident occurring?
(7) If no to (5), will the Minister request that an investigation take place concerning this matter?
(8) Can the Minister list for each individual concern from inspectors including all non compliances identified within the last 36 months at the above referred to operations?
(9) If no to (8), why not?
(10) In the period for the last 36 months can the Minister state how many prohibition notices have been issued by DMP inspectors for all the above operations and list all the reasons as to why they were issued?
(11) If no to (10), why not?
(12) In the period for the last 36 months, can the Minister state, -
(a) how many improvement and provisional improvement notices have been issued by DMP inspectors for all the above operations; and
(b) list all the reasons as to why they were issued?
(13) If no to (12), why not?
(14) Can the Minister quote the full text for all the mines record book entries, stop work orders, notices issued by inspectors at the above operations for the years, -
(a) 2008;
(b) 2009; and
(c) 2010?
(15) If no to (14), why not?

(b) The report submitted by the company indicates that the load haul dump operator failed to see the jumbo due to a blind spot on the right hand side of the machine and he was unable to stop before a collision occurred.
3. An inspector made inquiries on site on 11 May 2010 as to the circumstances of this incident and is satisfied that appropriate remedial actions have been taken.

(b)  A steering part failed leading to a collision with the decline sidewall.
