(1) What is the current number of prisoners on the At-risk Monitoring System at each facility, and of those: (a) how many are on high-level monitoring versus low-level monitoring; (b) how many have been continuously on the At-risk Monitoring System for longer than 14 days, one month, and three months; and (c) what is the average duration a prisoner remains on the At-risk Monitoring System? (2) What is the current number of prisoners in the Crisis Care Unit at each facility due to being at risk of self-harm, and of those: (a) what is the average length of stay in the Crisis Care Unit; (b) what is the minimum length of stay currently recorded; and (c) what is the maximum length of stay currently recorded? (3) For the financial years 2020–21 through to 2024–25, please advise, broken down by facility, the number of recorded incidents of: (a) self-harm; (b) attempted suicide; and (c) suicide ideation reports? (4) Of the incidents identified in (3): (a) how many related to prisoners already identified on the At-risk Monitoring System, by financial year; (b) how many occurred within seven days of a prisoner being removed from the At-risk Monitoring System, by financial year; and (c) how many required hospitalisation outside of the facility, by financial year? (5) What formal protocols exist to ensure timely case management and follow-up for prisoners identified as at-risk of self-harm, and how often are these protocols audited for compliance? (6) What performance indicators are currently used by the Department to measure the adequacy of mental health care and suicide prevention in WA prisons?
