I refer to the
Healthway sponsorship awarded to Perth Glory this year.
(1) On which
date did Healthway receive an application from Perth Glory for a sponsorship?
(2) Which
parties provided letters of support for Perth Glory's application?
(3) Did any
members of the Healthway board declare a conflict of interest in Perth Glory's
application for a Healthway sponsorship?
(4) If yes to

On behalf of the parliamentary secretary representing the
Minister for Health, I provide the following response —
(1) An
application for sponsorship from the Perth Glory Football Club was received on
29 February 2012. This application was subsequently deferred. In July 2012 it
was reported to the Healthway Sport and Racing Advisory Committee that the
application had been withdrawn pending further discussion and negotiations with
the organisation. A revised application from Perth Glory was received on 12 March
2013. This application was approved by the board of Healthway on 18 April 2013.
(2) Healthway
discourages organisations from including letters of support with sponsorship
applications. No letters of support were provided with the applications.
(3) No.
