(a) The subacute service model of care, also known as step-up, step-down, combines appropriately balanced clinical and nonclinical mental health services for people who are at risk of becoming unwell but do not require admission to hospital and for people leaving hospital who may require additional support before they return home.
The services provided by the Joondalup subacute service are underpinned by this model. The Joondalup subacute service provides person centred treatment and support to consumers within a framework of recovery and rehabilitation, through its optimal health program derived from the Collaborative Recovery Model.
The service adopts an integrated approach focusing on maintenance, engagement or reengagement in positive and supportive social, family, educational and vocational connections.
(b) 22 beds are provided at the Joondalup subacute service, which commenced operation in May 2013. Up to a further 50 beds are planned for other subacute services, comprising:
1. Rockingham: 10 residential beds
12 in-home care beds are under consideration
2. Broome: 6 residential beds
3. Kalgoorlie-Boulder: 6 residential beds
4. Bunbury: 10 residential beds
5. Karratha: 6 residential beds
