(d) how many cases were removed from the waiting list without surgery (tertiary and non-tertiary hospitals only) for each of the following categories:
(i) external audit (health service);
(ii) patient no longer wants procedure;
(iii) patient non contactable;
(iv) procedure no longer performed; and
(v) surgery no longer appropriate,
for each month from 1 July 2007 to 31 December 2007; and
(e) how many cases were removed from the waiting list without surgery (other removals) (tertiary and non-tertiary hospitals only) for each of the following categories:
(i) admitted as emergency for waitlisted condition;
(ii) duplicate entry;
(iii) patient had procedure elsewhere (e.g. attended GP or private hospital);
(iv) procedure performed during emergency admission for other condition; and
(v) transferred to another hospital,
