(1) Yes. May/June 2000. (2) Western All-Pest Services. (3) Contract price of installation. (4) Yes, albeit that no formal contract existed at the time, it having expired in 2001 (5) No 'Notices of Termicide Application ' were ever issued. No diagrams of the system were produced in spite of repeated requests. (6) Western All-Pest Services (7) Yes (a) John Mark Peacock T/A Naturalist Pest Control (b) & (c) 28/09/04, 14/10/04, 21/10/04, 11/11/04, 17/11/04, 30/11/04, 10/12/04, 21/12/04 and 14/01/05. (d) Qualified Pesticide Operator for the purpose of Division 5 of Part 11 of the Health (Pesticides) Regulations 1956. Licence no: 3188. Naturaliste Pest Control is registered with the Department of Health and is a member of the Australian Environmental Pest Managers Association. John Peacock has a Bachelor of Applied Science Degree majoring in Environmental Health. (8) Commencing October 2005. (9) Ultimately it would expose the site to the risk of white ant activity. However the pest control system is properly maintained annually.
