(1) I refer to incidents involving injuries to Department of Fire and Emergency Services (DFES) personnel or damage to DFES appliances, in relation to the burnover incident at Wooroloo in February 2021, and I ask: (a) in relation to a truck rollover near Boyanup in July 2022; (b) when and where did this incident take place; (c) did any DFES personnel sustain injuries and if so, what was the extent of the injuries; (d) did any DFES appliances sustain damage and if so, please provide detail; (e) has a safety investigation been completed by DFES or WorkSafe; and (f) if yes to (d) please table the investigation? (2) in relation to a 3.4 Urban Tanker rollover on the Minilya-Exmouth Road on 27 April 2023: (a) when and where did this incident take place; (b) did any DFES personnel sustain injuries and if so, what was the extent of the injuries; (c) did any DFES appliances sustain damage and if so, please provide detail; (d) has a safety investigation been completed by DFES or WorkSafe; and (e) if yes to (d), please table the investigation? (3) in relation to a 12.2 tanker rollover in the Perth metropolitan region in October 2023: (a) when and where did this incident take place; (b) did any DFES personnel sustain injuries and if so, what was the extend of the injuries; (c) did any DFES appliances sustain damage and if so, please provide detail; and (d) is DFES or WorkSafe conducting an investigation into this incident? (4) in relation to reports of a burnover event during the Mariginiup Bushfire in November 2023: (a) when and where did this incident take place; (b) did any DFES personnel sustain injuries, and if so, what was the extent of the injuries; and (c) is DFES or WorkSafe conducting an investigation into this incident? (5) since 1 January 2023, is the Minister aware of any additional incidents where DFES personnel or appliances sustained injuries or damage, and if so, please provide detail? (6) are 3.4 urban tankers or 12.2 tankers fitted with rollover protection systems, and if not, is DFES considering rollover protection systems for these appliances in future?

The Department of Fire and Emergency Services (DFES) advises: (1) (a-b) A Type 1 Urban Pump fire appliance rollover occurred on 26 July 2022 at 0127 hours on South Western Highway, 1.5 kilometres north of Boyanup. (c) The driver sustained a minor left shoulder injury, and soreness in the back. A passenger sustained minor lacerations to the rear and right-hand side of the head, and minor soreness in the neck, right shoulder, and lower back. (d) The Type 1 Urban Pump fire appliance was a loss. (e) DFES and WorkSafe investigated the incident. (f) See tabled paper no. XXXX
(2) (a) The incident occurred at 1347 hours on Miniliya-Exmouth Road, 130 kilometres south of Exmouth on 27 April 2023. (b) The driver sustained injuries to his right hand and arm requiring surgery. The passengersustained injuries to his ear and a minor tear to his left shoulder.
(c) The 3.4 SOSF Urban Tanker fire appliance was substantially damaged. (d) DFES is conducting an internal safety incident investigation. (e) Not applicable
(3) (a) On 14 October 2023, at 1435 hours on Chandler Road, Ashendon. (b)-(c) No. A Local Government Bush Fire Brigade (BFB) bulk water tanker applianceslipped on the shoulder of an unsealed gravel road and rolled onto the passenger side of theappliance. Approximately $100,000 in damage was done to the appliance. (d) DFES is not investigating this incident as it was a Local Government Appliance and a Local Government controlled bushfire. WorkSafe could not confirm if they were conducting an investigation.
(4) (a) On 22 November 2023, at 1800 hours, at 151 Mariginiup Road, Mariginiup. (b)-(c) No. DFES is conducting an internal safety incident investigation into this incident.
(5) Between 1 January 2023 and 30 November 2023, there have been no additional incidents to report.
(6) Rollover and fall-on protection systems are under consideration for future DFES appliances Maintaining axle masses within limits defined by Main Roads WA impedes the ability to include fall-on and rollover protection systems without losing water capacity.
