(1) WA Police do not collect additional information about the hospitalisation of those injured in traffic accidents beyond the initial determination of injury severity made by paramedics at the scene of the crash.
(2) The Minister for Transport has advised that hazards can be reported to Main Roads via their web site under faults or via the phone on 138 138. If the hazard is on a local government asset it will be forwarded through to the relevant local government authority for a response.
In terms of near misses there is no system and there is no system for near misses for any mode of transport.
