(a) The South West Home and Continuing Care include services such as Home and Community Care (HACC) and Transition Care Programs.
The region is divided into the sub-regions of Blackwood, Preston and Vasse.
The data below is for HACC services.
Data for 2013-14 will not be available until December 2014.
