I refer to the death of a ten year old child made reference to in question on notice No. 2920, and ask -
(1) What date did the child die?
(2) Have the investigations by the Ombudsman and the Coronial Inquiries commenced?
(3) If no to (2), why not?
(4) If yes to (2), what stage are the investigations up to?

(2) On 14 December 2010 the Ombudsman of Western Australia wrote informing the Department for Child Protection that the death of the child is an investigable death under section 19A(3) of the
Parliamentary Commissioner Act 1971
and a review will be conducted. The Coroner's Court of Western Australia notified the Department of the child's death on 15 October 2010. The child was under a time limited protection order, therefore a Coronial Inquiry is mandated.
