I refer to the response to Question on Notice 5478 where Schedule 8 medications were considered to be lost, and ask: (a) Was each incident of stolen Schedule 8 medications reported to WA Police for further investigation: (i) If not, why not; and (ii) If so, what was the date of each report to WA Police and what agency or health service provider was responsible for the initial report?
